Aim: Cadmium is a heavy metal pollutant that is a human carcinogen. Exposure occurs primarily through ingestion and inhalation. Materials and Methods: In our study, we examined the mortality rates, Disability Adjusted Life Years (DALYs) and population-attributable fraction (%) due to cadmium exposure in China and the world for the years 2000, 2010 and 2016 using the World Health Organization (WHO) occupational disease burden application tool. Results: In China, the mortality rate (per 100,000 of population) due to occupational cadmium exposure due to bronchus, trachea and lung cancer has not decreased over the years, and the mortality rate due to exposure is still high. According to the latest 2016 data, although the death rate (per 100,000 of population) in China (.4269) is higher than the world (.2002) average; the death rate (per 100,000 of population) was determined to be similar to Europe (.4650), but lower than the death rates in Africa (44.9514), Americas (30.0927), Eastern Mediterranean (12.5471), South-East Pacific (14.4381) and Western Pacific (201.1176) regions (p<.05). As a result of the comparisons made, it is seen that the number (for DALYs in 1000s) due to cadmium exposure has increased over the years (2000: 3.82, 2010: 4.95, 2016: 5.55). It was determined that the number (for DALYs in 1000s) showed significant differences over the years (p<.05). Mortality rates (per 100,000 of population) have been decreasing over the years (2000: 79.7906, 2010: 49.3388, 2016: 41.4298) (p<.05). Conclusion: Morbidity and mortality rates due to cadmium exposure remain high due to rapidly increasing industrialization.
Giriş ve Amaç: Sağlık sektörü, barındırdığı biyolojik, kimyasal, ergonomik ve psikososyal risk faktörleri nedeniyle iş sağlığı ve güvenliği (İSG) literatüründe “Çok Tehlikeli” sınıfa yaklaşan karmaşık bir yapıya sahiptir. Dünya Sağlık Örgütü (WHO) verilerine göre sağlık çalışanları, genel popülasyona kıyasla 3 kat daha fazla kas-iskelet sistemi rahatsızlığına maruz kalmaktadır. Türkiye’de ise artan iş yükü ve “bakım paradoksu” (health paradox), çalışan güvenliğini tehdit eden kritik bir sorun haline gelmiştir. Bu çalışmanın amacı, 2004-2024 yılları arasında Türkiye sağlık ve sosyal hizmetler sektöründe meydana gelen iş kazaları ve meslek hastalıklarını analiz etmek ve küresel karşılaştırmasını yapmaktır. Materyal ve Metot: Çalışma, retrospektif zaman serisi analizi tasarımında kurgulanmış olup; SGK İstatistik Yıllıkları, ILOSTAT ve EUROSTAT veritabanlarından elde edilen 20 yıllık zaman serisi verilerini kapsamaktadır. Verilerin analizinde iki temel istatistiksel model kullanılmıştır: • Kesintili Zaman Serisi (ITS): 6331 sayılı İSG Kanunu’nun (2013) ve COVID-19 pandemisinin (2020) etkilerini ölçmek için Poisson Regresyon modeli kurulmuştur. • Monte Carlo Simülasyonu: Meslek hastalıklarındaki eksik bildirimi (underreporting) tahmin etmek amacıyla 10.000 iterasyonlu bir stokastik simülasyon gerçekleştirilmiştir. Bulgular: Analiz sonuçları, 2013 yılındaki yasal düzenlemenin kaza bildirimlerinde istatistiksel olarak anlamlı (p < 0,001) bir yapısal kırılma yarattığını; pandemi döneminde (2020-2024) ise kaza sayılarında %22,1’lik üstel bir artış (şok) gözlendiğini göstermektedir. Meslek hastalıklarında %97,44 oranında bir eksik bildirim tahmin edilmiştir. Bu bulgu literatürde belirtilen “veri körlüğü” sorunu ile uyumludur. Biyolojik ve ergonomik riskler baskın olmasına rağmen, illiyet bağı sorunu nedeniyle kayıtlar düşüktür. Risk matriksi analizinde Türkiye, OECD ortalamasına kıyasla hem kaza sıklığında hem de ölümcül kaza oranında negatif ayrışmıştır. İşyeri şiddeti ise sağlık çalışanlarına yönelik AB ortalamasının 4 katına ulaşmış ve regresyon analizine göre kaza sıklığını açıklayan ikinci en güçlü faktör (R² = 0,65) olarak belirlenmiş olup, şiddetin iş sonuçlarına etkisi literatürle paralellik göstermektedir. Sonuç ve Öneriler: Bu çalışma, Türkiye sağlık sektörünün İSG açısından “risk yönetimi” aşamasına geçemediğini ve veri entegrasyonu eksikliği yaşadığını ortaya koymaktadır. Sektörde İSG standartlarının yükseltilmesi için; biyolojik ve ergonomik hastalıklarda illiyet bağı aranmaksızın “Meslek Hastalığı Karinesi” uygulanmalı, şiddet olayları iş kazası kapsamında raporlanmalı ve personel başına düşen hasta sayısı OECD standartlarına çekilerek iş yükü azaltılmalıdır.
Purpose: Alcohol consumption can cause death, various diseases and permanent disabilities. The harmful effects of alcohol consumption are a significant public health problem. Excessive alcohol consumption and alcohol dependence are conditions that cause significant health problems affecting 10% of the US and Europe. Methods: In this study, the most up-to-date data on alcohol-related death rates from the EUROSTAT website for the years 2011-2021 and from the TURKSTAT for the years 2011-2023 were used. EUROSTAT had data for 37 countries. The Microsoft Office 365 Excel program was used to calculate averages and create graphs. The relevant years are the years with the most up-to-date data. Results: According to EUROSTAT data, the countries with the highest alcohol-related death rates among 37 countries between 2011 and 2021 were Slovenia, Denmark, and Latvia. The lowest alcohol-related death rates were observed in Montenegro, North Macedonia, and Albania. It is observed that this rate is low in Türkiye. According to the 2011-2023 data of the Turkish Statistical Institute, while the alcohol-related death rate was high in 2011 and 2012, it decreased towards 2023. Studies conducted show that alcohol directly and indirectly causes death, and alcohol-related mortality rates are at a significant level. Conclusion: When reporting deaths, providing detailed explanations of the causes of death or supporting deaths with autopsy findings will be useful in identifying alcohol-related deaths. Intervention actions to reduce alcohol consumption should be developed, existing policies should be revised, or new policies should be created to reduce alcohol consumption.
Background/Aim: Although the global prevalence of smoking has been steadily declining, aggregate trends conceal substantial regional differences across Europe, as well as socioeconomic and education-level inequalities. The aim of this study was to assess the widening regional and educational inequalities in smoking prevalence in Europe and de-lineate structural barriers to tobacco control against the backdrop of aggregate trends. Methods: Using European Statistical Office (Eurostat) data from 32 countries (2014–2019), daily smoking rates, indoor environmental tobacco smoke exposure, and smoking intensity were analyzed. To account for demographic differences, European averages were calculated using population-weighted means. Pearson correlations and multivariate regression models were utilized to examine country-level associations between educational attainment indices and smoking outcomes. Results: While the weighted European Union (EU) average for daily smoking declined slightly, regional divergence intensified. Nordic countries maintained the lowest prevalence (~12%), whereas Eastern European nations and Türkiye (~27–29%) exhibited stagnation or increases. A strong country-level educational gradient persisted; nations with higher tertiary attainment rates exhibited significantly lower aggregate smoking prevalence. Marked cross-country differences in indoor secondhand smoke exposure were observed; daily indoor tobacco smoke exposure exceeded 19% in countries such as Greece and Croatia but remained below 3% in Finland. Conclusion: The decline in smoking has stalled in structurally disadvantaged regions, supporting the “hardening hypothesis” (the phenomenon where the remaining smoker population becomes more resistant to quitting) at the macro level. Generic tobacco control measures are losing efficacy. To bridge the widening health equity gap, future interventions must target high levels of heavy smoking and the enforcement of smoke-free environments in lagging regions.
Background: We aimed to determine the factors affecting the nutritional attitudes of female hospital workers toward their children between 9 months and 6 years of age during the COVID-19 pandemic.Methods: Our study, which is an original article, was conducted with female employees working in the hospital and having children between the ages of 9 months and 6 years. The impact of changes in work and home life during COVID-19 on changes in child care and child nutrition has been questioned. Result: Statistical significance was detected between the scale and questions such as the frequency of anxiety regarding the child's illness during the COVID-19 process, the decrease in the child's appetite, changes in the child's eating pattern, the mother's relationship with the environment and social relations, and the child's sleep pattern. Conclusion: A significant relationship was found between inadequate/unbalanced nutrition and educational status and between the reaction to others' opinions sub-dimension and age groups. Moreover, another significant relationship was found between the frequency of anxiety about the child's illness and the scale, and between the change in the child's nutrition pattern and the scale. The COVID-19 pandemic affected the attitudes of female employees towards the nutrition of their children.
This study is aimed to determine the ideal seismic gap distance that should be between the structures to prevent collisions with each other as a result of the displacements of two adjacent base-isolated structures under different earthquake effects. A total of three adjacent building models were created by adding structures with reduced mass and increased mass with the same characteristics of a base-isolation system. In that case, the uncertainty of variable loading of the structures is considered. The dynamic analyses of three different adjacent structures were carried out for two different periods of base isolation system and under different earthquake records. As a result of the analysis, the displacements and accelerations of the three adjacent structures were determined. The seismic gap distances between structures at different periods for the base isolation and with different masses were compared. It has been found that the maximum seismic gap distance among adjacent structures is between the structure with increased massed structure and the main structure. Also, it is high when the period for the base isolation is high.
Fuels for energy production and road traffic are the main causes of air pollution. According to 2020 data from the World Health Organization (WHO), the number of deaths from air pollution is 6.7 million per year. Ozone has harmful effects if it is above the limit value. Our aim is to investigate the effect of ozone on mortality when it exceeds the limit value at ground level. In our study, we used data from Aydın province, which has the highest average ground-level ozone level in Turkey. From the Air Quality Monitoring website of the Ministry of Environment and Urbanization, we collected data from 4 stations in Aydın province and divided by the number of stations to obtain the average ground-level O3 value. We used the World Health Organization AIRQ+ 2.2 program to calculate ozone-related mortality. Ministry of Environment, Urbanization and Climate Change Air Quality Monitoring website, we calculated the average O3 value as 64.73 μg/m³ between 01.03.2022-30.08.2022, which is the 6 months with the highest amount of O3 at ground level in Aydın province. According to the results obtained in the AIRQ+ program, the estimated attributable mortality rate was 0.47% (CI 95%, RR: 0-0.93), the estimated number of deaths from attributable cases was 22 (CI 95%, RR: 0-44) and the estimated number of deaths from attributable cases per 100000 population at risk was 1.92 (CI 95%, RR: 0-3.80). The long-term effect of ground-level ozone appears to be undeniable in increasing the number of deaths.
Giriş ve Amaç: Omurgalı hayvanlardan insanlara, insanlardan da hayvanlara geçen zoonotik hastalıklar halk sağlığını tehdit ederler ve sağlık hizmeti çalışanlarında mesleki risk oluşturmaktadırlar. Hekim adaylarının ve hayvan sağlığı alanında çalışanların zoonotik hastalıklarla ilgili doğru bilgi, tutum ve davranışlara sahip olması gerekmektedir. Bu araştırmanın amacı; bir Tıp Fakültesi 5 ve 6. Sınıf öğrencilerinin ve bir belediye çalışanlarının zoonozlarla ilgili bilgi, tutum ve davranışlarının saptanmasıdır. Gereç ve Yöntem: Bu çalışma, 01.10.2018 ve 31.07.2019 tarihleri arasında yürütülmüş tanımlayıcı bir araştırmadır. Araştırmanın evrenini bir Tıp Fakültesi 5 ve 6. Sınıf öğrencileri ve bir belediyenin[1] veteriner işleri çalışanları oluşturmaktadır. Öğrenciler 1000 kişi ve belediye çalışanları 437 kişidir. Bu evrenden örnek belirlemek için OpenEpi programı kullanılmış, %95 güven aralığında, ±%5 hata ile çalışmanın gücü %80, iki grup arasındaki bilgi düzeyi farkı % 15 olacağı varsayılarak, 271 öğrenci ve 136 belediye çalışanı ile görüşülmesi hedeflenmiştir. Anketler gözlem altında yanıtlama yöntemi ile uygulanmıştır. Çalışma için Klinik Araştırmalar Etik Kurul İzni alınmıştır. Bulgular: Beşinci sınıf öğrencilerinin %35i, altıncı sınıf öğrencilerinin %35,4ü, belediye çalışanlarının %23,1i hayvanlarla riskli temas öyküsü olduğunu bildirdi. Beşinci sınıf öğrencilerinin %99,3ü, altıncı sınıf öğrencilerinin tamamı, belediye çalışanlarının %92,6sı hayvanlardan insanlara hastalık bulaşabileceğini bilirken, beşinci sınıf öğrencilerinin %77,7si, altıncı sınıf öğrencilerinin %81,9u, belediye çalışanlarının %26,5i insanlardan hayvanlara hastalık bulaşabileceğini biliyordu. Beşinci sınıfların %21,3ü, altıncı sınıfların %21,5i, belediye çalışanlarının %40,9u o güne kadar kuduz aşısı yaptırmıştı; beşinci sınıfların %77,5i, altıncı sınıfların %82,5i, belediye çalışanlarının %23,8i tüberküloz aşısı yaptırdığını; beşinci sınıfların %91,3ü, altıncı sınıfların %78,5i, belediye çalışanlarının %86sı tetanoz aşısı yaptırdığını; beşinci sınıfların %3,6sı, altıncı sınıfların %3,5i, Belediye çalışanlarının %3,1i sarı humma aşısı yaptırdıklarını bildirdi. Beşinci sınıfların %12,2sinin, altıncı sınıfların %15,3ünün, belediye çalışanlarının %8,1inin çevresinde zoonoz geçiren olmuştu. Beşinci sınıf öğrencilerinin %87,1i, altıncı sınıf öğrencilerinin %85,5i, belediye çalışanlarının % 55,5i zoonotik hastalıklarla mücadele için evcil hayvanların yok edilmesini doğru bulmazken, sırasıyla yaban hayvanlarla evcil hayvanların ilişkisinin kesilmesinin zoonotik hastalıklarla mücadelede etkili olduğunu bildirenler %79, %91,7 ve %67,2 idi. Mesleki uygulamada karşılaştıkları zoonozlar bruselloz, şarbon, tüberküloz, toksoplazmoz, kuduz şüphesi, kuş gribi ve Kırım Kongo kanamalı ateşi idi. Sonuç ve Öneriler: Zoonozlar konusunda araştırma grubu bilgi düzeyleri %25-%99 arasındadır; hem öğrenci grubunun, hem de belediye çalışanlarının bu konuda eğitime ihtiyaçları vardır.
Structures are exposed to dynamic loads such as ground motions. Structures exposed to ground motion may be subjected to deformation if they do not have control systems. Control systems are used in structures to minimize these deformations. These control systems consist of types such as active, passive, semi-active, and hybrid. The application of these control systems varies depending on the type of structure and the ground motion to which it will be exposed. Since many variables are effective in the design of control systems, it is necessary to optimize these systems. Thanks to metaheuristic-based optimization, the best solution can be obtained under an objective function, as long as certain constraints are provided. In this study, optimization studies for active, passive, semi-active, and hybrid control systems are reviewed.
