Recommended obesity prevention interventions target multiple levels. Effective advocacy is needed to influence factors at individual, social, environmental, and policy levels. This paper describes the rationale for engaging youth in obesity prevention advocacy efforts targeting environment and policy changes to improve nutrition and physical activity. Advocacy involves education, skill development, and behavior and attitude changes, with the goal of persuading others or taking action. Youth advocacy has been successfully used in substance use prevention, but it is relatively new in obesity prevention. A model is presented to guide intervention and evaluation in youth advocacy for obesity prevention. With youth advocacy as a central construct, the model outlines inputs and outcomes of advocacy at individual, social environment, built environment, and policy levels. The model can be used and refined in youth advocacy evaluation projects. By involving youth in their communities, advocacy can produce ownership, engagement, and future involvement yielding sustainable changes.
From a cDNA clone bank prepared from cadmium-treated HeLa cells, we isolated clones representing mRNAs whose concentration is increased after cadmium induction. Several metallothionein cDNA clones were isolated by cross-hybridization to mouse metallothionein-I cDNA. The nucleotide sequence of one of these clones, containing a nearly full-length cDNA copy of human metallothionein-II mRNA, was determined. The homology between the human and mouse metallothionein sequences is strictly limited to the coding region of the mRNA. Codon usage in metallothionein mRNA is not random. Seventy-nine percent of the codons have G or C residues at the third position, resulting in a GC-rich sequence.
Walking for exercise is becoming widely recognized for bestowing health benefits. This study examined the association of walking for exercise and mood in sedentary, ethnic minority women over a five-month period. Ethnic minority women (N = 102) participated in a randomized, controlled trial of a 7-week behaviorally based telephone and mail intervention that promoted the adoption of walking for exercise compared to a non-behavioral minimal intervention. At 2-month post-test and 5-month follow-up, participants reported significant decreases in depressive mood and increases in vigor. Increase in walking over the course of the study was associated with change in vigor. Limited evidence was found to support a relationship between walking for exercise and mood improvement in ethnic-minority women.

City planning policies influence urban lifestyles, health, and sustainability. We assessed policy frameworks for city planning for 25 cities across 19 lower-middle-income countries, upper-middle-income countries, and high-income countries to identify whether these policies supported the creation of healthy and sustainable cities. We systematically collected policy data for evidence-informed indicators related to integrated city planning, air pollution, destination accessibility, distribution of employment, demand management, design, density, distance to public transport, and transport infrastructure investment. Content analysis identified strengths, limitations, and gaps in policies, allowing us to draw comparisons between cities. We found that despite common policy rhetoric endorsing healthy and sustainable cities, there was a paucity of measurable policy targets in place to achieve these aspirations. Some policies were inconsistent with public health evidence, which sets up barriers to achieving healthy and sustainable urban environments. There is an urgent need to build capacity for health-enhancing city planning policy and governance, particularly in low-income and middle-income countries.
Some attributes of neighborhood environments are associated with physical activity among older adults. This study examined whether the associations were moderated by driving status. Older adults from neighborhoods differing in walkability and income completed written surveys and wore accelerometers (N = 880, mean age = 75 years, 56% women). Neighborhood environments were measured by geographic information systems and validated questionnaires. Driving status was defined on the basis of a driver's license, car ownership, and feeling comfortable to drive. Outcome variables included accelerometer-based physical activity and self-reported transport and leisure walking. Multilevel generalized linear regression was used. There was no significant Neighborhood Attribute × Driving Status interaction with objective physical activity or reported transport walking. For leisure walking, almost all environmental attributes were positive and significant among driving older adults but not among nondriving older adults (five significant interactions at p < .05). The findings suggest that driving status is likely to moderate the association between neighborhood environments and older adults' leisure walking.
Background: Neighborhood environment attributes of walkability and access to recreation facilities have been related to physical activity and weight status, but most self-report environment measures are lengthy. The 17-item PANES (Physical Activity Neighborhood Environment Scale) was developed to be comprehensive but brief enough for use in multipurpose surveys. The current study evaluated test-retest and alternate-form reliability of PANES items compared with multi-item subscales from the longer NEWS-A (Neighborhood Environment Walkability Scale—Abbreviated). Methods: Participants were 291 adults recruited from neighborhoods that varied in walkability in 3 US cities. Surveys were completed twice with a 27-day interval. Results: Test-retest ICCs for PANES items ranged from .52 to .88. Spearman correlations for the PANES single item vs NEWS-A subscale comparisons ranged from .27 to .81 (all P < .01). Conclusions: PANES items related to land use mix, residential density, pedestrian infrastructure, aesthetic qualities, and safety from traffic and crime were supported by correlations with NEWS-A subscales. Access to recreation facilities and street connectivity items were not supported. The brevity of PANES allows items to be included in studies or surveillance systems to expand knowledge about neighborhood environments.
Neighborhood environment studies of physical activity (PA) have been mainly single-country focused. The International Prevalence Study (IPS) presented a rare opportunity to examine neighborhood features across countries. The purpose of this analysis was to: 1) detect international neighborhood typologies based on participants' response patterns to an environment survey and 2) to estimate associations between neighborhood environment patterns and PA.A Latent Class Analysis (LCA) was conducted on pooled IPS adults (N=11,541) aged 18 to 64 years old (mean=37.5±12.8 yrs; 55.6% women) from 11 countries including Belgium, Brazil, Canada, Colombia, Hong Kong, Japan, Lithuania, New Zealand, Norway, Sweden, and the U.S. This subset used the Physical Activity Neighborhood Environment Survey (PANES) that briefly assessed 7 attributes within 10-15 minutes walk of participants' residences, including residential density, access to shops/services, recreational facilities, public transit facilities, presence of sidewalks and bike paths, and personal safety. LCA derived meaningful subgroups from participants' response patterns to PANES items, and participants were assigned to neighborhood types. The validated short-form International Physical Activity Questionnaire (IPAQ) measured likelihood of meeting the 150 minutes/week PA guideline. To validate derived classes, meeting the guideline either by walking or total PA was regressed on neighborhood types using a weighted generalized linear regression model, adjusting for gender, age and country.A 5-subgroup solution fitted the dataset and was interpretable. Neighborhood types were labeled, "Overall Activity Supportive (52% of sample)", "High Walkable and Unsafe with Few Recreation Facilities (16%)", "Safe with Active Transport Facilities (12%)", "Transit and Shops Dense with Few Amenities (15%)", and "Safe but Activity Unsupportive (5%)". Country representation differed by type (e.g., U.S. disproportionally represented "Safe but Activity Unsupportive"). Compared to the Safe but Activity Unsupportive, two types showed greater odds of meeting PA guideline for walking outcome (High Walkable and Unsafe with Few Recreation Facilities, OR=2.26 (95% CI 1.18-4.31); Overall Activity Supportive, OR=1.90 (95% CI 1.13-3.21). Significant but smaller odds ratios were also found for total PA.Meaningful neighborhood patterns generalized across countries and explained practical differences in PA. These observational results support WHO/UN recommendations for programs and policies targeted to improve features of the neighborhood environment for PA.
73 This study evaluated the reliability and validity of the phone version of the 7-Day Physical Activity Recall (7-D PAR), a standardized interview. The in-person version of the 7-D PAR has been one of the most extensively studied self-report measures of physical activity and is shown to be valid and reliable. Efficiency in research would be improved if the 7-D PAR could be conducted via phone, but this version needs to be validated. Seventy-four adults (48 females, 26 males), ranging widely in age (18 to 67) and activity levels, were studied. Participants completed 7 days of activity monitoring using the Tritrac-R3D® (TT), an accelerometer which records movement in 3-dimensions. Participants completed activity questionnaires, and both versions of the 7-D PAR on separate days, in counter-balanced order. Reliability was assessed by comparing the phone and in-person 7-D PAR data for six matched days. Validity was examined by comparing 7-D PAR and TT data for 6 matched days. Intraclass correlations between the phone and in-person 7-D PAR total KKD (kcals*kg-1*day-1) were high (R=.957). Correlations of TT-assessed KKD were r=.413, p<.001 with the phone 7-D PAR and r=.445, p<.001 with the in-person 7-D PAR. Analyses of recall of minutes of physical activity at different intensity levels on the phone 7-D PAR and recorded by the TT indicate low correlations at “moderate” levels of activity (r=.153, p<.20), and higher correlations for“hard” (r=.508, p<.001) and “very hard”(r=.764, p<.001) activity. Correlations were similar for the in-person 7-D PAR (r=.112, p<.35; r=.665, p<.001; r=.663, p<.001 - “moderate”, “hard” and“very hard”, respectively). The phone and in-person 7-D PARs are highly reliable measures of physical activity. The validity of both the phone and in-person 7-D PARs, especially for reports of activity at higher intensity levels, is supported by the correlations with objective monitoring.
NF-kappaB is constitutively active in many solid tumors, including breast cancer. However, the role of NF-kappaB in breast carcinogenesis is unknown. Ikkalpha(AA/AA) "knockin" mice in which activation of IkappaB kinase alpha (IKKalpha) is prevented by replacement of activation loop serines with alanines exhibit delayed mammary gland growth during pregnancy, because IKKalpha activity is required for cyclin D1 induction and proliferation of lobuloalveolar epithelial cells. Given the role of cyclin D1 in breast and mammary cancer, we examined involvement of IKKalpha in mammary carcinogenesis induced by oncogenes or a chemical carcinogen, 7,12-dimethylbenz[a]anthracene (DMBA). The Ikkalpha(AA) mutation retarded tumor development in response to either 7,12-dimethylbenzaanthracene or the MMTV-c-neu (ErbB2/Her2) transgene but had no effect on MMTV-v-Ha-ras-induced cancer, although both oncogenes rely on cyclin D1. Strikingly, primary Ikkalpha(AA/AA)/MMTV-c-neu carcinoma cells exhibited diminished self-renewal capacity, resulting in the inability to establish secondary tumors. Ikkalpha(AA/AA)/MMTV-c-neu carcinoma cells underwent premature senescence when cultured under conditions used for propagation of mammary gland stem cells. Thus, IKKalpha is not only a regulator of mammary epithelial proliferation, but is also an important contributor to ErbB2-induced oncogenesis, providing signals that maintain mammary tumor-initiating cells. IKKalpha may represent a novel and specific target for treatment of ErbB2-positive breast cancer.
