Results from some of these cohort studies have provided an indication of how nutrition guidelines have been applied in clinical practice and which areas need to be improved. Others have generated hypotheses that will be (or are already being) tested in randomized studies.
In a large, contemporary, and multicenter sample of patients with sepsis in the emergency department, hourly delays in antibiotic administration were associated with increased odds of hospital mortality even among patients who received antibiotics within 6 hours. The odds increased within each sepsis severity strata, and the increased odds of mortality were greatest in septic shock.

In stable septic patients, the PCO2 gap is not correlated with global indexes of gut oxygenation. The interpretation of PCO2 gap is more complex than previously thought.
Measurement of cardiac output (CO) using minimally invasive devices has gained popularity. In 11 patients we compared CO values obtained using the bioreactance technique--a new continuous, totally non-invasive CO monitor--with those obtained by semi-continuous thermodilution using a pulmonary artery catheter. We obtained CO measurements at study inclusion and after any relevant change in hemodynamic status (spontaneous or during fluid challenge, inotrope or vasopressor infusions). There was a poor correlation between the two techniques (r = 0.145). These data suggest that caution should be applied when using bioreactance devices in critically ill patients.
