Abstract The development of advanced contrast agents for disease diagnosis imaging has become a crucial frontier in oncological research. Notably, the fluorescence imaging contrast agents based on aggregation‐induced emission (AIE) stand out due to their exceptional sensitivity, superior signal‐to‐noise ratio, technical accessibility, and cost‐effectiveness, making them particularly valuable for biomedical applications. By harnessing the intrinsic luminescent properties of AIE molecules, researchers can engineer nanoparticles through molecular modifications or composite formulations to enable dual‐modal and multimodal imaging capabilities for both in vivo and in vitro applications. Importantly, the synergistic integration of multiple imaging modalities provides complementary diagnostic information while facilitating precise image‐guided interventions, including photothermal and photodynamic therapies, for complete lesion eradication. This comprehensive review examines the pivotal role and recent progress of AIE luminogens (AIEgens) in multimodal imaging systems, detailing their applications in diagnostic imaging and therapeutic guidance. The article further elucidates structure–property relationships in material design, showcases groundbreaking developments, and discusses emerging trends in AIEgen‐based biomedical technologies.
