During the past 10 years a large number of epidemiological studies have been conducted especially in the U.S. to show the relationship between treatment with exogenous estrogens and the appearance of endometrial cancer. The increase in number of cases has been of 65-80% according to some authors and of 100-150% according to others while mortality because of endometrial carcinoma has not increased. In reality the magnitude of the relative risk is still uncertain and it is possible to find several recurring bias in many of the published studies such as a lack of consideration for other risk factors notably obesity diabetes hypertension and parity. Estrogen therapy especially in menopause should be of short duration low-dose with cyclic administration of estrogens and progestins; regular medical and histological checks are a necessity.
